Group Costumes Requests
Any neighborhood or non-profit can request multiple costumes for the children they serve. 10/31 Consortium will work diligently to provide as many costumes as possible (100% fulfillment is not guaranteed). Please review the necessary requirements and complete the form below to request group costumes.

REQUIREMENTS:
- One adult must represent the group. This person is responsible for completing the form below, compiling a list of costume needs, and picking up the costumes from 10/31 Consortium at an agreed-upon date and time.
- All children receiving costumes should be assigned a number by the group representative. 10/31 Consortium should not receive the names of any children receiving costumes.
- The group representative should send a list of all costumes requests in an Excel file or Google sheet. The list should include the child’s number, clothing size (SML format), and first and second costume wishes. All information must be included. Incomplete requests will not be filled.
- Costume wishes should be generic: Super Hero, Princess, Witch, Zombie, Something Scary, Something Funny, etc. We try to match exactly as often as possible; however, this requirement is due to our limited supply. For example, a child who wants to be Batman may receive another hero character; a child who wants to be Elsa may receive another princess character. This must be communicated to the children by the group representative before costume distribution.
- Costume Request forms must be received by October 1. The group representative will be notified via email if 10/31 Consortium will be able to provide costumes. Approved groups must then submit the corresponding Costume Needs list by October 10.
